Mihama FAC Rupture Information Page

Summary A piping rupture likely caused by flow accelerated corrosion and/or cavitation-erosion occurred at Mihama-3 at 3:28pm on August 9, 2004, killing four and injuring seven.  One of the injured men later died, bringing the total to five fatalities.  The rupture was in the condensate system, upstream of the feedwater pumps (diagram), similar to the Surry and Loviisa locations.  According to Japan's Nuclear and Industrial Safety Agency (NISA), the rupture was 560mm (22 in) in size.  The pipe wall at the rupture location had thinned from 10mm (394 mils) to 1.5mm (59 mils) .  Mihama-3 entered commercial service 28 years ago and is a 826 MWe (gross) Mitsubishi-built PWR located 200 miles west of Tokyo.  The AP reports that sections of the failed line were examined in 1996, recommended for additional inspections in 2003, and scheduled for reinspection August 14 (five days after the rupture).

Our sympathies go out to the family, friends, and colleagues of those killed or injured in this tragic accident.
